Stomach sleeping is rough on your body as it hyperextends your lower back and twists your neck. But, if that's how you sleep, and you don't plan to change that, you need a mattress that minimizes the damage. The key is "firmness". Stomach sleepers need a firmer mattress to keep their hips from sinking too far. Because, when your hips dip while you sleep, it arches your lower back and causes significant pain. If your mattress is too soft, your midsection sags into the mattress, and your spine curves unnaturally. A firm (or firmer) mattress keeps your body flatter and your spine closer to neutral/ideal alignment.
"Hybrid mattresses with pocketed coils" are often the best choice for stomach sleepers because they provide firm, even support without pressure points. Mattresses made with natural materials like latex work especially well, as the latex layers provide cushion for comfort without letting you sink. All-latex mattresses in a firmer layers of latex are also great if you want to you prefer to sleep on a mattress without coils. If possible, avoid memory foam, as it's typically too soft for stomach sleepers, letting your hips sink into the mattress while your shoulders and legs stay higher. That's a recipe for back pain and a terrible night's sleep. Not to mention, memory foam mattresses trap heat and cook you from underneath while you sleep, causing you to "sleep hot".
Here's the truth: stomach sleeping isn't ideal for your spine, and no mattress will fix that completely. But a firm, supportive bed made with quality materials (latex, coils, organic cotton) will reduce the strain. If you wake up with lower back pain, your mattress is probably too soft. If you wake up with neck pain, consider a very low or flat pillow. Or, you could try training yourself to sleep on your side instead. Thread count over 400 is mostly marketing. Manufacturers use multi-ply yarns and low-quality cotton to inflate numbers, creating sheets that are less breathable and durable than honest 300-thread-count long-staple cotton. What actually matters: fiber quality, weave, and GOTS-certified organic cotton.

Ensuring that your mattress has proper support is just as important as the mattress itself. The right support system extends your mattress's lifespan, maintains proper support, and can even affect your warranty coverage. Old school box springs were designed for innerspring mattresses but will likely change the feel for most mattresses made today. Foundations and platform beds both work well for natural mattresses - but only if the slats are built right: at least 2 inches wide, no more than 3 inches apart, and rigid (not flexible).
